Because “home” is where the patient resides, we provide hospice services not only in the home, but for patients who may be in the hospital, a nursing home or an assisted living facility. Our family-centered approach promotes open and honest communication. Our goals include making the patient as comfortable as possible; working with the patient’s primary care physician to develop a care plan; serving as a resource to the patient and family; providing information so that educated decisions can be made about care; and providing emotional, spiritual and counseling support for the patient and the family.
